Loan Application Checklist

Please provide the following items in order to complete your loan package:

1. Most recent two years tax returns, all schedules

2. Most recent two years W2's

3. Most recent 30 days pay stubs

4. Most recent two months asset account statements (all accounts, all pages; internet print-outs can only be accepted if they show full account number and account holder's full name)

5. Most recent two months retirement account statements (all pages)

6. Copy of driver's license(s)

7. Name and phone number of homeowner's insurance agent

8. Signed loan disclosures (will be provided by loan officer upon receipt of executed contract)

IF APPLICABLE:

VA LOANS: DD214 & Certificate of Eligibility for Veteran

REFINANCE LOANS: Copy of settlement statement from previous closing (original purchase or most recent refinance), Copy of property survey, Copy of most recent mortgage statement

OTHER:

-Copy of divorce decree, if applicable

-Copy of social security award letter, if applicable

This list is only a general guide.  Other items may be required depending on loan type and scenario.
